If you are unable to
keep your appointment, please
notify us as far in advance as possible.
Your courtesy will allow us to accommodate
another patient who
may need an appointment
at that time.

Patient Appointments........
Because of our medical
specialty, we prefer
that initial appointments be
made by referral
from your
primary care physician.
It is in your best interest that
we work closely with your primary care
physician in that they
will provide your post cardiac
care.
If needed, future appointments will be made at the
time of your initial
visit,
or
we will notify you
of your next appointment.
Please call 334/ 280-1527 to schedule your appointment.
New Patient Registration
On your initial visit to
Montgomery Cardiovascular Associates,
please arrive
30 minutes before
your scheduled appointment time
to complete registration.
In order to reduce your
registration time on the day of your appointment, please

on and print the
New Patient Packet.
Please read
the
paperwork and fill
out the forms prior to your scheduled appointment. Obtaining
patient
information, medical history, and other pertinent information
can be time consuming.
Your help
and cooperation is greatly appreciated.
Please bring the following items
on the day of your appointment :

Any paperwork that has been mailed to you or downloaded

All medical insurance
cards so that we can properly file your
insurance claims

Your photo identification ( i.e. Drivers
License )
